Check out for any message in Google webmaster tool, Google sends message if it penalizes (not always). Or you might be witnessing Google dance.

Major reason could be stale content or you might have unintentionally violated Google guidelines.

Check the duplication in the website thoroughly if there then request to the Google and may be it will bring back your keywords after accepting the request.
