Default Need help with a form script on website

hi
I have a chiropractor client who would like people to be able to make appointments on her website. I see the forms out there - have not created one myself - would like to know what you experts recommend - Any suggestions?

Is this something I could purchase from a warrior? do myself simply or purchase from an outside opt in vendor? They seem simple enough - however I am not sure about this.

I believe your client just want a form where people can enter information and schedule appointment to him.

It's a simple form with all the necessary fields, including date and time, of course good user interface is highly recommended.

I just want the user to fill out the form, click the submit button and have the data sent to the server, and then be redirected to a "your form has been successfully processed" page.
