How do you reduce spam in blog comments ?

I've given up blog comments myself, but maybe some of these new tools can help to reduce blog comment spam ?

Has anyone used Disqus (for comments) on their site ? I see quite a lot of websites using this - anyone have experience with how much manual work you need to do to remove spam with this tool ?
