Creating "contact us" page on service website

Hi,

I have a service website, it has contact us page but it doesn't have contact details, it has only a firm to fill up...on my website e07.net

what would be suitable and effective for contact-us page, should I put my company address, email id and phone number for better result.

I see people visit contact us page and leave that page, they don't send any message through fill-up form.

Please guide me for better results.

You have to provide your valid company address and contact number for business purpose that people may contact you for business.

Of course, the company information should be contained, and service provider, email address, phone number if possible, and tell the visitors what you can do for him!
